Enhanced Care Management (ECM) offers extra services at no cost to Medi-Cal members who have complex needs and challenges that make it difficult to improve their health. This could include health conditions or challenges such as not having a place to live. To get ECM services, someone must be enrolled in a Medi-Cal managed care health plan and meet certain eligibility requirements. The ECM provider will assign a lead care manager who works with the client to help them get the health care and social services needed. The lead care manager helps the client:

This resource is eligible for electronic referral. Organizes volunteers to make minor home repairs or modifications which are health or safety related including installation of grab bars and hand rails, repair or replacement of electrical fixtures and leaking drains, help with ramps, and minor repairs to entry steps and porches. Modifications support people with disabilities, including seniors, so that they may continue to live in their own homes. 

This program is offered at our Main Office site in Grass Vallely AND at our Branch Office site in Yuba City.

To report a hate incident or hate crime, anonymously if you wish:
  • call 833-866-4283 (833-8NO-HATE) - you may speak to a live person Mon-Fri from 8am-5pm Pacific Time
  • or report online at https://stophate.calcivilrights.ca.gov/s/. Additional info and links to many resources are also on this page.
Whether or not you report anonymously, your identity will not be disclosed. The only exception to non-disclosure is if a report is made of child abuse, elder abuse, or activities indicating an imminent risk of violence.

The primary goals of this program are to ensure that victims and witnesses have access to needed services and that hate crimes and incidents get reported.
