Connecting Point offers Disaster Case Management for Nevada County residents impacted by the 2023 Severe Winter Storms (FEMA Incident Period February 21-July 10, 2023).

Call 211 or 833-342-5211 to receive a referral to a Disaster Case Manager who will contact you within one business day to offer assistance during your recovery process.

Disaster Case Managers will work one-on-one with individuals and families that have been impacted by the severe winter storms.

Meeting regularly and as needed, the case managers will be able to work with you step-by-step throughout the recovery process, provide you with resources for any need that comes up, navigate the County, State, Federal home repair and financial assistance processes that you will encounter, advocate for additional resources as needed, provide a physical place where you can print, fax, email, and copy documents, and keep you up to date with any updates as they are released.

PSPS is a Public Safety Power Shutoff event. Pacific Gas & Electric (PG&E) may do a PSPS when danger of wildfires is high. PSPS Care Coordination is a program to help PG&E customers who have health and medical needs that require electricity.

Care Coordination helps to get a plan in place for resources that can be used during a PSPS power outage event. Callers will receive a follow up call from Connecting Point staff so they can create their plan. The care coordination call will typically take 30-45 minutes.

Who qualifies?

Those who have access & functional needs, and/or are electricity dependent during a power outage (examples include medical equipment or medications that rely on power, accessibility equipment that relies on power, etc).

Placer County Public Health Department (PCPHD) partners with medical care providers in the community, and the California Department of Public Health, to prevent and control the spread of infectious diseases. We investigate and follow-up on reported cases and their contacts.
