Enhanced Care Management (ECM) offers extra services at no cost to Medi-Cal members who have complex needs and challenges that make it difficult to improve their health. This could include health conditions or challenges such as not having a place to live. To get ECM services, someone must be enrolled in a Medi-Cal managed care health plan and meet certain eligibility requirements. The ECM provider will assign a lead care manager who works with the client to help them get the health care and social services needed. The lead care manager helps the client:
  • Stay engaged in their health care
  • Make a plan to meet their health goals
  • Connect and update doctors about the clients' needs and wishes
  • Learn how to better support their health issues
  • Move safely from one care setting to another

Nation's Finest was awarded a FEMA (Federal Emergency Management Agency) grant to assist people affected by wildfires in Nevada and Placer Counties.

Anyone impacted by wildfires in Nevada or Placer County can receive free referral support, group support, and one-on-one crisis intervention by reaching out to 530-461-0257 or adecker@nationsfinest.org. Peer workshop appointments are available.

The Crisis Counseling Assistance and Training Program (CCP) assists individuals and communities in recovering from the effects of a Presidentially declared major disaster through community-based outreach and psycho-educational services.

In some cases, thieves try to apply for FEMA assistance using names, addresses and Social Security numbers they have stolen from survivors.

  • Federal and local disaster workers do not solicit or accept money. Don’t believe anyone who promises a disaster grant in return for payment. 
  • U.S. Small Business Administration representatives never charge applicants for disaster assistance, inspections or help in filling out applications.
