SENIOR OUTREACH NURSE - NCDSS

The Senior Outreach Nurse program provides health services to county residents 60 years and older. Offers comprehensive health and safety assessments, health screenings, fall prevention education, medication education/management resources, and continued in-home follow-up. In-home assessments review a client's medical, social, and environmental needs and offer health education consistent with client's needs. Will discuss referrals that are appropriate for clients, and help link them to other community resources and services. Provides follow-up visits and telephone calls as needed.

Offers outreach such as community based blood pressure clinics, flu shots in conjunction with Public Health, and participate in local health/wellness programs. Flu shots can be given to home-bound seniors. Call 530-265-1639 to arrange.

Senior Outreach Nurse does NOT do in-home COVID-19 TESTING. People who need in-home testing will need to get a home test kit.

The Senior Firewood Program provides qualified seniors a limited amount of pine/fir to supplement their other home heating sources when wood is available. Also accepts donations of wood as able.

PLEASE NOTE: Anyone wanting to apply to receive wood or donate wood should call to learn the current status of the program. Wood supplies generally run out each season and there are not always enough volunteers to accept wood donations. Call 530-615-4541 to learn when applications open and whether donations are currently being accepted.

At times when the Senior Firewood Program is out of wood, Project Go may be able to help both Nevada & Placer County residents if they are income eligible for LIHEAP (they do not need to be seniors, just income qualified). Call 916-782-3443… [read more]

Meals on Wheels provides qualified home-bound seniors over the age of 60 with nutritious lunches. Must be enrolled in the Meals on Wheels program. Delivery is on Tuesdays & Thursdays (no deliveries on weekends, major holidays or when roads are unsafe to travel). Gold Country Senior Services oversees the Meals on Wheels program in Western Nevada County (Alta Sierra, Grass Valley, Lake of the Pines, Lake Wildwood, Nevada City, Penn Valley, Rough and Ready, etc).*
