SENIOR OUTREACH NURSE - NCDSS

The Senior Outreach Nurse program provides health services to county residents 60 years and older. Offers comprehensive health and safety assessments, health screenings, fall prevention education, medication education/management resources, and continued in-home follow-up. In-home assessments review a client's medical, social, and environmental needs and offer health education consistent with client's needs. Will discuss referrals that are appropriate for clients, and help link them to other community resources and services. Provides follow-up visits and telephone calls as needed.

Offers outreach such as community based blood pressure clinics, flu shots in conjunction with Public Health, and participate in local health/wellness programs. Flu shots can be given to home-bound seniors. Call 530-265-1639 to arrange.

Senior Outreach Nurse does NOT do in-home COVID-19 TESTING. People who need in-home testing will need to get a home test kit.

GoldCountry Senior Firewood Program loses its wood yard

GRASSVALLEY, Calif. May 15, 2024 – On Monday, Gold Country Senior Services (GCSS)learned their lease on the Brunswick Road property, owned by Rise Gold, isbeing canceled early. The non-profit uses the site to store, split anddistribute firewood to low-income seniors. GCSS must vacate the property byJune 25.

Thesudden cancellation of the lease, means that the distribution days, typicallyscheduled for September, October and November, will not happen this year.Program participants are being asked to come get their wood now if they can.

GCSS isfaced with two urgent needs. First, they must find a new, long or short-termlocation for this program. Second, they must remove and deliver all theexisting wood before June 25. The County of Nevada is collaborating with GCSSto find a solution.

Meals on Wheels provides qualified home-bound seniors over the age of 60 with nutritious lunches. Must be enrolled in the Meals on Wheels program. Delivery is on Tuesdays & Thursdays (no deliveries on weekends, major holidays or when roads are unsafe to travel). Gold Country Senior Services oversees the Meals on Wheels program in Western Nevada County (Alta Sierra, Grass Valley, Lake of the Pines, Lake Wildwood, Nevada City, Penn Valley, Rough and Ready, etc).*

Gold Country Senior Services has been providing senior nutrition programs for older adults in Western Nevada County since 1987. They provide hot nutritious meals in a warm and friendly setting that provides older adults an opportunity to socialize. Each meal is prepared fresh and from scratch.

They do not serve meals on holidays or when roads are unsafe to travel.
Menu
Recipes and menus are planned and approved by a dietitian to ensure each meal meets the nutritional requirements for an older adult. Each meal includes an entrée, vegetable, fruit, and milk. Menus are available on their website and at their Café.
