Project HEART provides CalAIM services for eligible Medi-Cal members through Enhanced Care Management (ECM) and Community Supports (CS). This program is designed to meet people where they are; especially those facing complex challenges such as housing instability, medical needs, behavioral health barriers, or difficulty navigating support systems. Project Heart's goal is to reduce crisis cycles and improve stability through coordinated, person centered care.

Through ECM, participants receive individualized case management and care coordination that connects health, housing, and social supports into one plan. Project HEART helps clients access the services they need, communicate with providers, stay connected to appointments, and overcome barriers that often prevent progress. Project Heart provides a Project New Start program which offers a supportive pathway for individuals transitioning from incarceration back into the community. Project HEART’s goal is to help people rebuild their lives, reduce the risk of reoffending, and create a strong foundation for long-term stability. Project Heart walks alongside those who have faced life destabilizing challenges, providing guidance, encouragement, and practical support every step of the way.

Purpose is to break the cycle of incarceration by providing hope, structure, and community to those at high risk of reoffending; especially individuals whose past behaviors were driven by addiction, trauma, or lack of support.

Project Heart believes that everyone deserves a second chance, and that with accountability, real friendship, and access to resources, long-term change is possible.

FREED's Landlord Liaison Program (LLP) helps people experiencing or at risk of homelessness access and maintain stable housing.

Services include:

  • Assistance with first month’s rent and security deposits
  • Temporary rental subsidies
  • Credit-building support and limited debt repayment
  • Home accessibility modifications (smoke detectors, mobility aids, appliances)
  • Landlord incentives to increase housing opportunities for people on the HMIS By-Name List

No income limits apply. Program participation begins after enrollment in the coordinated entry system.

The 211 Connecting Point Contact Center provides a coordinated entry system for people currently experiencing homelessness.

Coordinated Entry callers will speak to a 211 Consumer Service Representative (CSR), who will walk them through a series of questions. The caller’s answers are entered by the 211 CSR into HUD’s Homeless Management Information System (HMIS), which can be accessed only by designated organizations that provide housing services.

Callers have access to emergency shelter, as well as long term/permanent housing, by going through the Coordinated Entry process.

Coordinated Entry intakes can be completed by calling 211's Homeless Resource Helpline at 833-3PLACER (833-375-2237).

The SAFE (Stability, Access, Foundation, Empowerment) program is committed to finding and helping youth ages 12 -25 who are experiencing homelessness.

Many youth experiencing homelessness do not have access to local or state funding or simply don’t know what services are available. And few nonprofit organizations in the region can help.

SAFE fills the gap by connecting youth who have alternative, unpredictable, or inconsistent housing access with community resources and well-trained professionals.

SAFE helps mitigate homelessness in Nevada County by providing:

  • Basic needs, such as food, clothing, housing, hygiene supplies, showers
  • Education and training, including transitioning to college and/or trade school
  • Physical health and mental wellness counseling
  • Training to develop the necessary skills to live independently

The Salvation Army's Booth Family Center provides a transitional housing program for families with children in Nevada County.

Services include:

  • Providing short-term emergency shelter for families with children
  • Offering transitional housing with flexible length of stay
  • Conducting intake interviews and maintaining a waitlist
  • Offering case management and counseling services
  • Supporting residents with education, employment, financial planning, parenting, substance use recovery, and mental health

Applicants must complete an online application form to join the Booth Family Center waitlist.

FREED provides assistance in finding accessible and affordable housing.

Services include:

  • Help in developing housing search strategies
  • Information about Section 8 and other subsidized and low income housing
  • Information and referrals to other community housing resources
  • Help in filling out apartment application forms
  • Assistance in understanding your housing rights as a person with a disability

FREED also offers a weekly Housing Club that provides information, resources and assistance for seniors and people with disabilities (any age) seeking support finding housing.

The Housing Club meets every Monday from 2-4pm and every Wednesday from 10am-12noon.. No appointment is needed.
