Chapa-De's Diabetes Program invites participants in their group classes and education programs that focus on the prevention and management of diabetes. As a department, it is their mission to help patients establish health goals and provide you with tools and support people's need to accomplish those goals. 4-H is an educational program of the University of California Cooperative Extension and is made available through a partnership of county, state, and federal governments.

The program creates positive opportunities for youth to develop life skills and become engaged in their local communities. The program combines the efforts of youth, volunteer leaders, and University staff and faculty to create a fun, hands-on learning approach to a non-formal education. Chapa-De provides Pregnancy Care at both Auburn and Grass Valley Health Centers.

In addition to regular appointments with your doctor, their Baby Luv Program helps women get the education and services needed for a healthy pregnancy and healthy baby.

Chapa-De's services include:

  • Check-ups at Chapa-De once a month or more until your 34th week, where you would then start having weekly appointments at Chapa-De with OBGYNs from Camelia Women’s Health who can deliver your baby at Mercy San Juan Medical Center
  • Tests to check your health and the health of your baby
  • Classes and one-on-one information about pregnancy, child birth, baby care, and breastfeeding
  • Referral to community agencies that can provide additional help and services
  • Information about where to go for financial help, health insurance like Medi-Cal, or food programs like WIC or local food banks
  • Help with cutting down or quitting smoking, drinking, or using drugs

Chapa-De Substance Use Disorder (SUD) Services are available to all American Indian community members and active medical patients at Chapa-De Indian Health. Addiction is a chronic, progressive, relapsing disease that can be treated. At Chapa-De, there is non-judgmental and compassionate care for all addictive diseases.

Services are provided in Auburn and Grass Valley.

The team at Chapa-De provides:

  • Routine screenings
  • Education
  • Brief interventions
  • Referrals to treatment for drug and alcohol addiction
  • Referrals to all levels of care, including outpatient treatment, intensive outpatient treatment, and residential treatment, which are available to patients with Medi-Cal. There are additional treatment options available for American Indian patients.

Youth Empowerment Support (YES!) is a program that supports youth and young adults through direct support, training, groups and leadership/system change opportunities.

YES! is a program of Whole Person Learning (WPL), a public agency that empowers youth and young adults to thrive in their communities by using a collaborative, individually tailored positive youth development approach.

Youth Coordinators help to accomplish goals by providing information and referrals for resources, identifying support systems, empowerment, education and advocacy, placement options. Program is focused on filling in the gaps when youth or young adults cannot access other supports to meet their identified needs. Support is voluntary and is offered on an as needed basis.
