The Alzheimer's Outreach Program (AOP) is a community service provided by Sierra Nevada Memorial Hospital that helps support any individual suffering from memory impairment or dementia and/or their caregiver. There are two components to AOP: The educational and clinical components.

What is the educational component of AOP?
AOP works toward providing educational opportunities for the community to become more informed about Alzheimer’s/Related dementias. AOP provides talks at each “Essential Tools for Caregivers” Series of classes - on “Caring for the Caregiver” as well as “Getting Directives in Place.” There are also collaborations with the Alzheimer’s Association for educational opportunities as well as collaborations with the Hospital Foundation for classes - both for caregivers and for professionals (hospital staff).

The Senior Outreach Nurse program provides health services to county residents 60 years and older. Offers comprehensive health and safety assessments, health screenings, fall prevention education, medication education/management resources, and continued in-home follow-up. In-home assessments review a client's medical, social, and environmental needs and offer health education consistent with client's needs. Will discuss referrals that are appropriate for clients, and help link them to other community resources and services. Provides follow-up visits and telephone calls as needed.

Offers outreach such as community based blood pressure clinics, flu shots in conjunction with Public Health, and participate in local health/wellness programs. Flu shots can be given to seniors in their homes. Call 530-265-1470 to arrange.

An identification database is maintained should an at-risk person be missing or is located. This program enables the police department's communications center to provide helpful information to patrol units and assist in locating a caregiver.

Founded in 1987, Del Oro Caregiver Resource Center is part of a statewide system of resource centers serving family caregivers. Del Oro covers California’s Gold Country Counties — Alpine, Amador, Calaveras, Colusa, El Dorado, Nevada, Placer, Sacramento, San Joaquin, Sierra, Sutter, Yolo, and Yuba — and is an independent, private, non-profit program funded by the Department of Health Care Services, Agency on Aging, Area 4, grants and private contributions.

Del Oro is dedicated to serving families caring for an adult with a cognitive impairing condition that occurred after the age of 18 (e.g., Dementia, Alzheimer's disease, stroke, traumatic brain injury, Parkinson's disease, and frail older adults) in various counties.
