Placer County Adult System of Care (ASOC) is a provider for Enhanced Care Management (ECM) and Community Supports Services (CSS). ECM provides comprehensive case management to Medi-Cal beneficiaries with the goal of linking people to important services such as healthcare, mental healthcare, housing services, and benefit programs. The goal of the Community Support Services programs is to help people find and maintain stable housing.

The target populations served by Placer ECM and CSS are adults over the age of 21 who:

  • are experiencing homelessness, and/or
  • have a serious mental illness (SMI) and/or a substance use disorder (SUD)

To learn more about ECM and CSS you can go to the State’s web page or contact your managed care plan. If you would like to be referred to ECM and/or CSS contact your Managed Care Plan. Partnership HealthPlan of California, and Kaiser Permanente (for people who were with KP as their managed care provider BEFORE1/1/24) are ECM/CSS providers in Placer County.

To stay at Volunteers of America Family Shelter, a referral through the Coordinated Entry system is required. To receive a referral, call the Placer County Homeless Resource Helpline at 833-375-2237 (833-3PLACER).

Volunteers of America Family Shelter, Home Start Inc. is a non-profit emergency shelter (operating 24/7, 365 days a year) for homeless families in Placer County.

Blossom Place provides safe housing, one-on-one support, and resources so former foster and at-risk young women ages 18-24 can learn life skills, achieve their goals, and gain the confidence to blossom into who they are meant to be.

Blossom Place believes one's basic needs of housing, food, clothing, and safety need to be met before you can move on to accomplish other goals. The Independence Plan is a step-by-step plan that moves a young woman through basic needs and into goals and dreams. With a mentor, a counselor, and a Blossom partner who acts case manager alongside her, she will work through education, career, health, transportation, and relationship goals. She'll learn time management, budgeting, cooking, cleaning, stress management, and parenting skills. The ultimate goal is for each girl to have all the pieces of the puzzle in place, and all the skills and the confidence in herself to successfully move on to independent living.

MEDICAL RESPITE PROGRAM - TGI AUBURN

In 2009 Sutter Roseville, Sutter Auburn Faith, and Kaiser, collaborated with the Gathering Inn, creating the Interim Care Program (ICP). Now called the Medical Respite Program, it provides people experiencing homelessness with a safe, clean place to rest and recover following hospital discharge. Services include:

  • Case management
  • Resource navigation
  • Mental health services

211 cannot provide a direct referral however, 211 can provide information so that people may ask to speak with their case manager or social worker at the hospital in order to receive a referral to the Medical Respite Program.

When open - The shelter will be open if it’s wet/raining at 37 degrees F. or below or open on the 4th night of consecutive rain. Additionally, they require two overnight hosts to open for overnight use. The shelter will open around 5 pm and close at 8 am the following morning. All shelter participants are asked to vacate the premises during the day.

Free hot meal - In addition to offering a warm, dry place to sleep, a free hot meal will be served from about 5-8pm and breakfast in the morning. Movies will be available to watch in addition to games.

Dogs - Dogs are permitted as long as they’re well-behaved, on leash, and with their persons at all times.

The Emergency Day Respite Center provides:
