Public Health (PH) Nurses are Registered Nurses who have additional training and certification to work in the community and the field of Public Health. Our goal is to meet preventive health needs of families and individuals, and promote positive health behavior. Our nurses offer in-home care to new parents, children, and the elderly.

If you or someone you know could benefit from Public Health Nursing assistance, start by submitting a Public Health Nursing Referral Form (PDF) to our office. Once received, our nurses contact the referred individual to offer services.

MEDICAL RESPITE PROGRAM - TGI AUBURN

In 2009 Sutter Roseville, Sutter Auburn Faith, and Kaiser, collaborated with the Gathering Inn, creating the Interim Care Program (ICP). Now called the Medical Respite Program, it provides people experiencing homelessness with a safe, clean place to rest and recover following hospital discharge. Services include:

  • Case management
  • Resource navigation
  • Mental health services

211 cannot provide a direct referral however, 211 can provide information so that people may ask to speak with their case manager or social worker at the hospital in order to receive a referral to the Medical Respite Program.

In 2005, Placer County Systems of Care with the support of the Mental Health Services Act (MHSA), joined forces with a host of community partners to launch a campaign to transform mental health services through community-based, recovery focused, culturally competent and innovative approaches. Believing the key to lasting change in mental health care in Placer County is to place residents and community based agencies in a true position of partnership with the county, the Campaign for Community Wellness (CCW) was created.

The Placer CCW functions as the legislatively mandated body to oversee the implementation of the Mental Health Services Act. The CCW is comprised of concerned community members, non-profit agencies, school and law enforcement partners, family members and consumers of Mental Health services and the Placer County Systems of Care staff. Self-referred individuals or families will either have an appointment scheduled with the Managed Care Unit, Sierra Mental Wellness Group, a Marriage and Family Therapist (MFT) or a Licensed Clinical Social Worker (LCSW) in the Placer County Network, or they will be linked to another appropriate community or agency provider depending on needs and insurance coverage. If you have questions you may call 530-866-5400.

Kaleidoscope of Employment for Youth Success - KEYS is a collaborative project between the Placer County Office of Education Foster Youth Services (FYS), the Department of Rehabilitation, and PRIDE Industries. This program is designed to ensure successful employment outcomes for foster youth, ages 16-19, who have significant barriers to employment.​​

Placer County foster youth, ages 16-19 years old, are matched with individual Job Developers who provide pre-employment support and assistance with obtaining and retaining employment local in a field of interest.

Eligible youth must have a significant barrier to gaining employment, including a mental health diagnosis, physical disability or special education designation. Proof of eligibility will be requested.

Harmony House is an adult residential facility with a treatment component program that offers mentally ill individuals a group living environment. Adults live at Harmony House cooperatively, learn the skills necessary to live independently while receiving support as needed from highly trained Mental Health Workers.
 
Harmony House is located in the foothills of Auburn, California in the hub of other Placer County departments. Many services are centrally located for residents and can be reached within walking distance.

Residents are selected from the in-patient population of acute psychiatric facilities, state hospitals, skilled nursing facilities, or from the community. Prospective residents are referred to Harmony House through Placer County Department of Mental Health.
