Sprout Organization operates a Recuperative Care program which provides a safe, recovery-focused environment for medically stable individuals experiencing homelessness or housing instability following hospitalization, illness, or injury. Services include:

• Weekday RN oversight
• Daily wellness monitoring
• Meals and basic necessities
• Case management
• Transportation coordination
• Housing navigation
• Connections to medical, behavioral health, and community-based services
