The program focuses on early intervention, resource navigation, and connecting individuals to appropriate community providers. Staff work directly with residents to identify needs, reduce barriers to care, and promote long‑term stability within Habitat housing communities.

Services provided:

• Behavioral health navigation - Helps residents connect with mental health providers, counseling, substance-use services, and community wellness programs
• Stability support - Assists individuals whose behavioral health needs may impact their ability to maintain housing
• Case management - Provides goal-setting, follow-up, and coordination with outside agencies
• Crisis support and problem solving - Offers immediate, short-term assistance during behavioral health-related challenges and helps residents access urgent resources
• Community resource linkage - Connects residents to transportation, food, medical care, employment services, and other supports that contribute to overall well-being
