The Alzheimer's Outreach Project (AOP) is a community service provided by Sierra Nevada Memorial Hospital that helps support any individual suffering from memory impairment or dementia and/or their caregiver. There are primarily 2 components to AOP: The educational and clinical components.

What is the educational component of AOP?
AOP works toward providing educational opportunities for the community to become more informed about Alzheimer’s/Related dementias. AOP provides talks at each “Yes I Can” Series of classes - on “Caring for the Caregiver” as well as “Getting Directives in Place.” There are also collaborations with the Alzheimer’s Association for educational opportunities as well as collaborations with the Hospital Foundation for classes- both for caregivers and for professionals (Hospital staff).

What is the clinical component of AOP?
AOP provides in home or on the phone social work assessments of need. We look at the whole picture from a biological, sociological and psychological standpoint and attempt to intervene and address concerns and issues, including:
  • Caregiver stress/burnout-linkage to mental health supports
  • Short and long range planning/ Help with placement decisions
  • Linkage to resources that may provide respite for caregivers/offer respite funds if appropriate.
  • Education/linkage on finding help in the home
  • Linkage to getting an accurate diagnosis
  • Education on managing difficult behaviors
  • Education on effective communication strategies for those with Alzheimer’s/Related dementia
  • Linkage to information on Caregiver Support Groups/Educational Opportunities
  • Facilitation of family meetings regarding planning, safety and stress
  • Education on importance of getting affairs in order (DPOA health/finance)
  • Linkage to Medi-Cal/IHSS
